Facilitation

 

PHOCUSED plays a unique role in the nonprofit community.

We provide an independent perspective from that of direct service providers while maintaining the close community connections that enable us to bring those service organizations and concerned individuals together. This collaboration is necessary to produce social change and contribute to a robust health and human services sector.

From 2015–2017, PHOCUSED participated with Partners In Care (PIC), Oʻahu's Continuum of Care, through a grant from Aloha United Way and facilitated case conferencing for single adults experiencing homelessness.

Additionally, through a generous opportunity provided by the Hawaii Community Foundation’s HousingASAP Program, PHOCUSED served as the Coordinated Entry System (CES) Administrator for families experiencing homelessness. During this one-year contract, PHOCUSED executed the U.S. Department of Housing and Urban Development-mandated process, which allows resources to be more effectively directed to families most in need.

As the CES administrator for families, PHOCUSED worked with family service and housing providers to match and provide referrals for families. Family CES was conducted in collaboration with PIC and Bridging the Gap, the Balance of State Continuum of Care.

PHOCUSED also formerly served as a liaison between the Department of Human Services’ ‘Ohana Nui initiative and the non-profit community.
